We will need you to fill out this questionnaire.

We will also need to talk with you once we receive your questionnaire. It is not that we are trying to be nosey but we want to be sure that we place the right pup that will fit your lifestyle. Not all breeds are right for every household,even though all puppies are cute. =)

Please read our health guarantee visiting us and Health Concerns page before filling this out. otherwise, when you coming back you'll have to fill it out all over again!
